First: Are You Automatically Enrolled?
Before filling out any forms, check if you even need to apply. You are automatically enrolled in Medicare Parts A and B if:
- You are already collecting Social Security retirement benefits at least 4 months before your 65th birthday.
- You have received Social Security Disability Insurance (SSDI) checks for 24 months.
- You have Amyotrophic Lateral Sclerosis (ALS / Lou Gehrig's disease).
If you qualify for automatic enrollment, you do not need to do anything! Your official Medicare card will automatically arrive in your mailbox about 3 months before your 65th birthday.
Who Needs to Manually Apply?
You must actively apply if:
- You are turning 65 and have not yet started collecting Social Security benefits (for example, if you plan to delay Social Security until age 67 or 70).
- You are over 65, delayed Medicare because you had active employer group health coverage, and are now preparing to retire.
- You do not qualify for premium-free Part A and wish to purchase coverage.
The 3 Ways to Apply for Medicare
Method 1: Apply Online at SSA.gov (Fastest & Recommended)
The easiest way to apply is online through the Social Security Administration portal (even though Medicare is healthcare, the SSA processes all enrollment applications):
- Step 1: Go to the official portal at www.ssa.gov/medicare.
- Step 2: Click the blue button: "Apply for Medicare Only" (or "Apply for Retirement and Medicare" if taking both).
- Step 3: Log in using your Login.gov or ID.me credentials.
- Step 4: Answer basic personal, employment, and citizenship questions. The application takes approximately 10 to 15 minutes.
- Step 5: Submit electronically. You will receive an instant confirmation number to track your application status.
Method 2: Apply by Phone
If you prefer speaking with a live federal representative or need assistance, call the Social Security Administration toll-free at 1-800-772-1213 (TTY: 1-800-325-0778) Monday through Friday, 8:00 AM to 7:00 PM local time. The representative can complete your application over the phone or schedule an appointment.
Method 3: Apply In Person
You can visit your local Social Security field office. We strongly recommend calling ahead to schedule an in-person appointment to avoid hours of waiting in line. Use the official SSA Office Locator at secure.ssa.gov/ICON/main.jsp to find your local branch.
Applying Past Age 65: The Special Enrollment Forms
If you worked past age 65 with employer group coverage and delayed Part B, you must submit two official federal forms to avoid late penalties:
โข Form CMS-40B: "Application for Enrollment in Medicare Part B" (Completed by you).
โข Form CMS-L564: "Request for Employment Information" (Completed and signed by your employer's HR or benefits department proving you had continuous active health coverage).
What Happens Next?
Once your application is submitted, you can track its progress by logging into your account at ssa.gov/myaccount. Approval typically takes 2 to 4 weeks, after which your official Medicare card and welcome packet will arrive in the mail.
